Located in Limeport, Pennsylvania, New Vitae Mount Trexler Manor is one of two licensed Personal Care Home options within the New Vitae Wellness and Recovery continuum of residential care. Mount Trexler Manor is adjacent to a fruit orchard, providing an opportunity for personal reflection while still being central to shopping, movies, and other activities. The home has a capacity for 90 residents and is licensed by the Department of Human services. New Vitae Mount Trexler Manor provides safety and structure with 24 hour staffing while also encouraging each resident’s progress toward recovery or other goals.
 
New Vitae Mount Trexler Manor provides local transportation to appointments and three nutritious meals and snacks daily. The dedicated, professional staff assists with interest assessments in order to pair individuals based on preferred hobbies to encourage socialization and to build peer relationships. In this and other ways, Mount Trexler Manor is a warm and welcoming community that provides an environment that encourages personal growth and change.

Medication Administration
Medications are administered through the efforts of trained staff who provide assistance and education regarding medication.

Holistic Care
The Director of Wellness is available to assist with the development of healthy living skills, along with the rest of Mount Trexler Manor.  Staff members provide support at medical appointments for any pre-existing or new health concerns.

Individualized Supports
Staff members work closely with each resident to review options for services that include community groups, volunteer opportunities, and other choices to determine what services may work best. Each resident has a Resident Assessment-Support Plan (RASP) to guide his or her services.

Spirituality Supports
Mount Trexler Manor is centrally located in order to provide routine visits to local churches, synogogues, mosques, and faith centers. A visiting chaplain is also available to engage those with an interest in developing their spiritual beliefs and practices.

Vocational Assistance
Residents of Mount Trexler Manor have the opportunity to work in our Prevocational Work Program.  The program offers the opportunity for work shadowing and assistance while developing a resident’s resume to prepare for competitive work options.  Our staff will also work with residents for referral to the Office of Vocational Rehabilitation (OVR).

Routine Trips
Staff members encourage participants to utilize the frequent trips that permit community connections. Visits to local 12-Step support groups, shopping centers, museums, parks, concerts, and other local attractions occur routinely in order to encourage additional peer-to-peer contact and socialization.

Peer-led Opportunities
Emphasizing the importance of peer contact, residents have the opportunity to participate in the Mount Trexler Manor Resident Council.  In this way, residents can provide direct feedback to staff and other leaders regarding their experiences at Mount Trexler Manor.  It also permits development of leadership skills for the residents who choose to participate.

House Sponsored Events
Mount Trexler Manor emphasizes the importance of community by hosting various events for residents.  Culture days, dances, games, crafts, basketball games, baseball games and holiday parties are enjoyed by staff and residents. Day trips to Philadelphia, New York, and a yearly trip to Cape May, New Jersey are additional ways to explore and connect through personal interests.

Benefits/Care Coordination
Mount Trexler Manor provides care coordination services to ensure continuation of benefits, to encourage community connections, and to provide personalized supports throughout residents’ participation in services.

Family Day Activities
Family members, friends, and supportive allies are always welcome to visit. Mount Trexler Manor also offers larger events that include training opportunities, picnics, and seasonal activities, connecting our residents with their families in a fun and supportive environment.
